What is P-Tau?

Alzheimer’s disease is a complex condition, but ongoing research is giving us powerful tools to detect changes in the brain earlier than ever before creating real opportunities to take action and live well. The earlier potential issues are identified, the sooner individuals can focus on proactive steps that support brain health and overall quality of life. One important discovery is the biomarker phosphorylated tau, or p-tau, which is helping shift the focus toward prevention, early awareness, and personalized care.

In a healthy brain, tau proteins play a critical role in stabilizing the internal structure of neurons and supporting the transport of essential nutrients within brain cells. In Alzheimer’s disease, tau can become chemically altered—a process called phosphorylation—causing it to malfunction and form tangles inside neurons. These tangles can disrupt communication between brain cells and contribute to damage over time.

P-tau has become an important biomarker because it can signal Alzheimer’s-related changes in the brain sometimes decades before symptoms appear. Because of this strong connection, measuring P-tau helps track disease progression and identify individuals who may be at increased risk for Alzheimer’s earlier than other methods. In May of 2025, the U.S. Food and Drug Administration cleared for marketing the first in lab diagnostic device that tests blood to aid in diagnosing Alzheimer’s disease. “Alzheimer’s disease impacts too many people, more than breast cancer and prostate cancer combined,” said FDA Commissioner Martin A. Makary, M.D., M.P.H. “I am hopeful that new medical products such as this one will help patients.”

While p-tau testing offers promising insight into brain health, “A positive p-tau test is not an Alzheimer’s diagnosis,” explains Dr. Bergman. “It is just one biomarker that must be evaluated alongside the whole person and other clinical findings.”
